Preparation sites in Pakistan publish chapter rankings constantly, and almost none of them say where the numbers came from. This page is the counter-example: every figure below is counted from papers we hold and link to, the method is stated in full, and the subjects the evidence cannot reach are named rather than quietly filled in. You are welcome to reuse any of it with attribution.

Finding 1: the paper is far more concentrated than the syllabus suggests
Across the 76 chapters this analysis can measure, 17 chapters account for half of all MCQs (91 of roughly 180 marks), and 41 account for 80%. The syllabus presents every chapter as equal. The papers do not.
This is the practical finding for a student with six weeks left: revision time spent from the top of this list returns more marks per hour than time spread evenly across the syllabus.

Finding 2: real repetition is rarer than the market claims
Searching all 8,806 questions for the same question with the same correct answer in two or more different exam years found 233 repeats. The most any single question has come back is 4 exam years.
That number is deliberately unflattering. Pages promising “the 50 most repeated ETEA MDCAT MCQs” are common, and on this corpus the honest count is what it is. A repeat is a fact about papers: without the papers there is no claim to make, and with them the answer is checkable. Every repeat we list names the years and papers it was found in.

Method and limits
The corpus. 48 solved past papers, 2005 to 2025, containing 8,806 questions that resolve to a published MCQ in our bank. The papers are ETEA MDCAT, MDCAT, NUMS, and every one of them is readable on this site.
Chapter distribution. Each chapter’s share of its subject is counted from those papers, then scaled to what the subject is worth on the paper as it is set today. The share is the part that holds steady across pattern changes; the scale comes from the conducting body’s current weightage. Reporting raw historical averages instead would misreport every subject whose weightage has since changed.
Repetition. A repeat is the same question with the same correct answer in two or more different exam years. Matching on question text alone would group unrelated questions that share a generic stem; matching on the full text including options would miss questions that came back reworded. Two papers from the same year using one question counts once.
What this cannot tell you. These are historical distributions, not a prediction of one specific paper. A chapter averaging eight MCQs can come up with five.
