A question counts as repeated only when the same question, with the same correct answer, appears in two or more different exam years. Matching is done on the question text and the correct answer together, so a question that came back with its options reworded or reordered is still caught, while the dozens of unrelated English questions that share a stem like “Choose the correct option” are not counted as repeats of each other.
