How does the USMLE route work for a Pakistani MBBS graduate?

The short answer

The route runs: ECFMG registration and degree verification, then USMLE Step 1 (basic sciences, pass/fail since 2022), then Step 2 CK (clinical knowledge, scored, now the number programmes rank on), an English requirement (currently OET Medicine or an alternative on the ECFMG list), ECFMG certification, then US clinical experience, letters of recommendation and the residency match, with Step 3 during or after residency. For a Pakistani graduate the realistic timeline is two to four years from deciding to starting residency, and the budget runs to several thousand US dollars in fees before travel and living costs for electives and interviews. Since Step 1 became pass/fail, a strong Step 2 CK score, recent graduation, US experience and research are what the match rewards; Step 1 only has to be passed, on the first attempt.

What are the stages, in order?

  1. Register with ECFMG and verify your degree

    Obtain a USMLE/ECFMG identification number, then have your MBBS verified through ECFMG's credential verification (via the EPIC portfolio for most applicants). Your medical school must be listed in the World Directory of Medical Schools with an ECFMG sponsor note. Verification can take months; start it first.

  2. Step 1

    One day, seven blocks of up to 40 questions, about eight hours, at a Prometric centre (there are centres in Pakistan; availability varies). Basic sciences applied to clinical vignettes. Pass/fail, and a fail on the record is a serious mark in the match, so sit it only when practice scores are comfortably above the pass line.

  3. Step 2 CK

    One day, nine hours, clinical knowledge across medicine, surgery, paediatrics, obstetrics and gynaecology, psychiatry and preventive medicine. Scored on a three-digit scale, and this is now the score programmes filter and rank on.

  4. English or communication requirement

    ECFMG currently accepts OET Medicine at grade B in each sub-test, with other pathways published from year to year. Check the ECFMG site for the requirement in force for your certification year; see OET explained.

  5. ECFMG certification

    Issued once Step 1, Step 2 CK, the communication requirement and credential verification are complete. It is what makes you eligible to enter the match and to hold a US training licence.

  6. US clinical experience, research and letters

    Observerships, externships or electives in the US, ideally in your intended specialty, produce the letters of recommendation programmes want. This is the stage where cost and visas bite, and where a graduate outside the US is most disadvantaged.

  7. The match

    Apply through ERAS in September, interview from October to January, rank programmes in February, match in March, start in late June or July. Programmes filter on Step 2 CK, year of graduation, visa need, US experience and letters.

  8. Step 3

    Two days, taken during residency in most cases, and required for a full licence. Some visa categories (H-1B) require it before residency starts, which is worth knowing when choosing programmes.

Step 1 content, by system and discipline, in broad terms
  • Pass/fail means the goal is a comfortable first-attempt pass, not a score. The test is no easier; the pass line sits around the level a well-prepared graduate reaches with several months of dedicated study.
  • Preparation has shifted towards Step 2 CK. Graduates now spend less time perfecting Step 1 and more on clinical knowledge, and many sit Step 2 CK first while the clinical years are fresh.
  • A fail is visible. Programmes see attempts. Delay the sitting until a full-length practice assessment predicts a pass with a margin; the ECFMG and NBME self-assessments are the honest measure.

A typical sequence for a recent graduate, in broad terms
  • Fees: ECFMG registration and verification, Step 1, Step 2 CK, OET, ERAS applications (priced per programme, and international graduates apply to many), the NRMP match fee, and Step 3 later. Several thousand US dollars in fees alone.
  • Travel and living: electives and observerships in the US for weeks to months, and interview travel, though many interviews are now virtual. This is usually the larger cost.
  • Visa: J-1 sponsored by ECFMG is the common route and carries a two-year home residency requirement after training; H-1B needs Step 3 first and a sponsoring programme.

What does the match actually reward?

  • Step 2 CK score is the main filter now that Step 1 is pass/fail.
  • Year of graduation. Many programmes screen out graduates more than three to five years out. This is the single biggest reason to start early or choose another route.
  • US clinical experience and US letters. A letter from a US attending in the specialty carries weight a home letter does not.
  • Research, especially for competitive specialties.
  • Visa need. Programmes that do not sponsor visas filter international graduates out regardless of scores. Apply to those that do.
  • Specialty choice. Internal medicine, family medicine, paediatrics, psychiatry, pathology and neurology take international graduates in numbers; surgical subspecialties, dermatology and radiology rarely do without exceptional profiles.

How do you prepare for Step 1 from Pakistan?

  1. Build on a single comprehensive review source and one question bank

    The standard combination is a systems-based review text with a large clinical-vignette question bank, worked through twice. Adding more sources adds time, not marks.

  2. Do questions from the first week, not after reading

    Step 1 vignettes test application. Reading a chapter then doing its questions, and reviewing every explanation including the wrong options, is the method; passive reading is not.

  3. Use the basic-science banks to shore up weak subjects

    Where a subject was weak at MBBS, rebuild it at recall level first: pathology, physiology, pharmacology. Then return to vignettes.

  4. Sit official self-assessments at fixed points

    One a month in the last three months. Book the exam when two consecutive assessments predict a pass with margin.

  5. Start Step 2 CK material as soon as Step 1 is done

    The score that matters is the next one, and the clinical knowledge is freshest immediately after house job.

Is USMLE Step 1 pass/fail?

Yes, since January 2022. The exam is unchanged in difficulty; only the reporting changed. A fail is visible to programmes, so the goal is a comfortable first-attempt pass, and the Step 2 CK score has become the number programmes rank on.

How long does the USMLE route take from Pakistan?
