How does the AMC route to practising in Australia work?
The short answer
For most Pakistani graduates the route is the AMC standard pathway: verify your primary qualification through the AMC portal and EPIC, meet the English requirement (IELTS Academic 7.0 in each component, OET grade B, PTE 65 in each or TOEFL equivalent), pass the AMC CAT MCQ examination, a computer-adaptive paper of 150 questions in three and a half hours on clinical medicine at the level of an Australian graduate, then pass the AMC Clinical examination (a multi-station clinical assessment) or complete a workplace-based assessment in an accredited hospital. With the AMC certificate you apply to the Medical Board for provisional registration, work under supervision for at least twelve months, then general registration. The exams are passable with six months of preparation; the hard part is securing the supervised position, which is why many international graduates start in regional and rural hospitals.

Expedited specialist routes
Australia has been introducing expedited recognition for specialists from selected countries in shortage specialties. It does not currently apply to Pakistani qualifications directly, but a specialist with UK, Irish or similar training may qualify. Check the Medical Board's current list.

- Adaptive means you cannot skip and return. Each answer is committed. Pacing is by question count, not by section.
- The clinical standard is Australian: Australian guidelines, PBS drug choices, the Australian public health context (indigenous health, rural medicine, immunisation schedules). Those are the gaps for an overseas graduate.
- Basic sciences appear as mechanisms behind management. The pathology, pharmacology and physiology banks cover the layer beneath the vignettes.
What is the AMC Clinical examination, and what is the alternative?
- AMC Clinical examination: a multi-station structured clinical assessment (traditionally sixteen scored stations) with simulated patients, examining history, examination, diagnosis, management and communication across the same disciplines as the MCQ. Held at the AMC's centre in Melbourne, with demand exceeding places, so bookings are competitive.
- Workplace-based assessment (WBA): an alternative for candidates who hold a supervised position in an accredited Australian hospital; assessment over about six months in the workplace replaces the clinical exam. It requires the job first, which is the same bottleneck.
- Either route yields the AMC certificate, which the Medical Board requires for general registration after the supervised period.
How do registration and the first job work?
Verify the degree
Create an AMC portal account and have the MBBS verified through EPIC. Do this first; it takes months.
Meet the English requirement
IELTS Academic 7.0 in each component, OET grade B in each sub-test, PTE Academic 65 in each, or TOEFL with the Board's component minimums, within two years at registration, with combining across two sittings allowed under conditions. See OET explained.
Pass the MCQ examination
Then apply for the Clinical examination or seek a position for WBA.
Secure a supervised position
Hospitals advertise intern-equivalent and resident positions; international graduates most often find them in regional, rural and outer-metropolitan hospitals and in shortage specialties. The position is what makes provisional registration possible.
Provisional registration and supervised practice
Granted by the Medical Board of Australia through AHPRA for the specific position. At least twelve months of supervised practice with satisfactory reports.
General registration
Applied for after the supervised period and the AMC certificate. From here, specialty training is by application to the relevant college, and general practice by the RACGP or ACRRM pathways.
The job is the bottleneck
Passing the MCQ is achievable with six months of preparation. Passing it and then spending a year unable to secure a supervised position is the common experience. Apply widely, consider regional and rural hospitals from the start, and understand that a visa (usually employer-sponsored) is tied to the position.
How do you prepare for the MCQ?
Learn the Australian clinical context
Australian therapeutic guidelines, the immunisation schedule, the public health priorities and the general practice model. This is the gap that fails otherwise strong candidates.
Work a large SBA bank in clinical vignettes
Four to six months of daily blocks, every explanation read. Note the Australian-specific management points in one list.
Practise committing to answers
Adaptive testing does not allow review. Timed practice where you cannot go back trains the discipline.
